The club takes safety seriously and having a safe culture requires members to submit their concerns regarding safety.

Members can discuss or report their concerns in several ways depending upon how you would like it handled and reviewed. You are welcome to submit your concerns anonymously however it is beneficial for a reviewer to come back to you to ask further questions to clarify details.

You may submit any concerns, issues, incidents or near misses via this form. The information will be sent to the president, vice president and safety officer for review. Please include as much information as you can to assist in improving the club’s culture, processes and activities. You can look at the incident reporting form to help provide as much detail as possible. Alternatively you can report issues through using the club’s incident report form, which will help structure relevant information.

Please note: if you choose to remain anonymous, the club's actions may be limited. We will try to take as much informal action as we can with the information given to us, but for a larger response we encourage you to provide more details/contact information. We will always maintain confidentiality if you do choose to provide us with your details.


Members may wish to discuss their concerns directly with a committee member such as the sports officer, the safety officer, president or vice president.
